Granite Wellness: Road to Resilience

Program Spotlight

We are excited to share a new collaborative no-cost program available across all Granite Wellness Centers’ (GWC) campuses and surrounding communities. Road to Resilience (aka Resilient Mom) supports pregnant women and families with a child under 1 with substance exposure. This program aims to help moms have a healthy pregnancy, healthy babies, and overall family wellness. Once referred, a navigator is assigned to partner with mom to support her in achieving individual and family wellness and a positive quality of life. Currently, home visitations are limited; however, navigators can communicate, educate, and meet mom’s needs via phone or zoom. Road to Resilience is a community-based home visitation program that is not specific to GWC.

The Road to Resilience program connects mothers to Navigators from local community-based organizations. Navigators partner with moms to plan for and achieve a healthy pregnancy and welcome a healthy, happy baby. Moms can also receive on-going support as their babies grow and will get connected to services, such as:

 Basic needs support
 Community supports
 Early dental screenings
 Medical care
 Early childhood literacy programs
 Parenting support and classes
 Substance use support services
 Mental health services
