Children With Special Health Needs

The Family Health Outcomes Project (FHOP) is collecting feedback from parents of Children and Youth with Special Health Needs in California.  This includes “those who have, or who are at increased risk for, a chronic physical, developmental, behavioral, or emotional condition.  They also require health and related services of a type or amount beyond that generally required.”  Though the survey is focused on children who receive services through California Children’s Services (CCS), all parents of a child with special health needs are encouraged to complete the survey.

Please feel free to distribute this information to those who have children who meet the above definition, so we can collect information from a variety of community members.  Information from the survey will be used to plan for future services available to Children and Youth with Special Health Needs.  The survey will take approximately 20 minutes to complete.  Here are links to the survey in English and Spanish:
